Home Support Service (Palliative Care)

INTRODUCTION

Louth & District Hospice and Age Concern - Lindsey have joined forces to provide a Home Support Service for people with palliative care needs.

People with palliative care needs are those people who have a disease that cannot be cured. Some of these people may be in the last few months of their lives.

Sometimes practical help and support is needed during this time to help those affected to cope with every day life.

WHAT DOES THE SERVICE PROVIDE?

The Home Support Service (Palliative Care) is not a nursing or personal care service but it will undertake the following activities:

  • Domestic house work and cleaning
  • Preparing light meals and drinks
  • Shopping and picking up pensions
  • Sitting service
  • Laundry (washing and ironing)
  • Gardening - light work

WHEN IS THE SERVICE AVAILABLE ?

All the services, apart from the sitting service, will be provided 9am - 5pm Monday to Friday. The sitting service will be offered during the day or the night, 7 days a week, subject to availability.

HOW MUCH DOES THE SERVICE COST?

All the services are free at the point of service for any person with palliative care needs.

HOW DO PEOPLE GET THE SERVICE?

Only district and community nurses can refer people to the service.

If you are not sure who the appropriate nurse is for a particular person, contact their GP’s surgery.

WHAT HAPPENS AFTER A REFERRAL IS MADE?

An Age Concern Scheme Manager will undertake an assessment of the individual’s needs.

Once the service has been agreed Age Concern will leave a pocket file in the person’s home with contact numbers and details of the service to be provided.

Initially the service with be offered in the following areas:

Caistor Spilsby
Chapel St Leonard Sutton-on-Sea
Holton-le-Clay Tealby
Horncastle Woodhall Spa
Louth Wragby
Mablethorpe
